How do I complete a form?

Click on  New form  and select the desired form. 

 Enter at a minimum the mandatory fields. 

 Select the location(s) where the form should be displayed. 

 Click on  Save and close  to create the form. 

 Enter at a minimum the mandatory fields. 

 Click on  Save & close.
